Our home
CarePlus Health Care Services LLC operates Moses & Christiana Group Home, an APD-licensed residential group home in Clearwater, Florida. Our home gives adults with developmental disabilities and complex care needs a safe, supportive and structured place to live. We are RN-owned and operated, with a strong focus on dignity, independence, health, safety and quality of life.
Meet Rene Parra, RN, BSN, CCM
Rene Parra is a Registered Nurse with over 18 years of healthcare experience and the Owner and Administrator of CarePlus Health Care Services LLC. Her background spans hospital case management, home health, utilization management and direct patient care, so she understands the needs of people with developmental disabilities and complex medical conditions.
Rene founded Moses & Christiana Group Home out of a passion for serving vulnerable people: a place where residents can thrive, keep their dignity and grow more independent. As an APD-licensed provider, she is committed to high-quality care that supports health, safety, community life and personal growth.
Her mission is simple: compassionate care, safe living and supportive growth, helping each resident reach their fullest potential.

A simple, supportive path to admission
We work closely with families, Waiver Support Coordinators and case managers to make referrals and admissions as smooth as possible.
Submit a referral
A family member, WSC or case manager contacts us by phone or email with the prospective resident's information.
Assessment & review
Our team reviews medical, behavioral and support needs to confirm we are the right fit for the person.
Coordination
We coordinate with the WSC, case manager and family to confirm services, funding and a move-in date.
Welcome home
The new resident arrives to a personal, person-centered support plan in place from day one.
